A Case of Cold Urticaria with Exercise- and Cold-Induced Anaphylaxis

Abstract

Few cases who developed food-dependent exercise-induced anaphylaxis in winter have been reported. However, no case has been reported thus far which developed an anaphylactic reaction to an exercise challenge in cold urticaria. We experienced one such case and report here with the first case of cold urticaria with exercise and cold induced anaphylaxis that was confirmed with challenges. A 24-year-old man who was atopic showed a positive reaction to an ice cube test and a negative methacholine intradermal test. Five minutes after a free running test for 6 minutes at about 20oC, large hives, dizziness, and hypotension developed. He did not show an airflow obstruction after exercise. Atopics having an organ sensitive to a stimulus, i.e., local anaphylaxis, may develop systemic anaphylaxis when they are exposed to stronger stimuli.
