Arthroscopic Superior Capsule Reconstruction for Irreparable Massive Rotator Cuff Tear

Abstract

An arthroscopic superior capsule reconstruction for irreparable massive rotator cuff tears in relatively young patients restored the force couple using graft materials fixed on the greater tuberosity and the glenoid leading to shoulder function improvement. A fascia lata autograft and acellular dermal matrix allograft are the most commonly used graft materials. An autograft of fascia lata has the advantage in thickness and no foreign body reaction compared to allograft. On the other hand, donor site complications and longer operation time are disadvantages for fascia lata autograft. The clinical outcomes have been generally satisfactory despite a 3%–70% retear rate and a 0%–5% postoperative infections rate. This article reviews the recent studies regarding indications, surgical techniques and clinical outcomes of various graft materials for superior capsule reconstruction and introduces the author’s preferred technique.
