The effect of Pyrazinamide on Uric Acid Metabolism

Abstract

It is well known that pyrazinamide inhibits renal excretion of urate, but we could not find clinical gout frequently. Therefore we have studied on the effect of pyrazinamide therapy in a daily dose of 1.5 gm on the urate metabolism in 29 active pulmonary tuberculosis patients (male 17 and female 12), who had normal kidney function and no metabolic disorders. on a regular hospital low-purine diet. 1) The serum concentration of urate were 4.4±1.9 mg/ dl before treatment and 6.5 ±1.7mg/ dl after pyrazinamide therapy for 5 days. 2) The rate of urate clearance to creatinine cleance (C urate/ Ccreatinine) were 0.11 ± 0.07 before treatment and 0.07 ± 0.04 after pyrazinamide therapy for 5 days. 3) The ratio of urine urate to urine creatinine were 0.54± 0.25 before treatment and 0.38± O.l7 after pyrazinamide therapy for 5 days. The mechanism of pyrazamide induced hyperuricemia should be studied further.
