Combined Abduction and Derotation Osteotomy in Bilateral Congenital Coxa Bara Vara: Report of Bilateral Treatment

Abstract

Treatment of congenital coxa vara is surgical. Its aim is to correct the decreased femoral neck-shaft angle. There are two standard procedures available to achieve this aim; Haas sub-trochanteric osteotomy is followed by gradual correction by skeletal traction, while Blount and Cleveland employ blade plate and spline which is bent to be predetermined angle. However, these methods pose considerable limitations in its application and are often technically difficult. In addition, neither of these methods mention correction of outward torsion of the femur seen in congenital coxa vara. It was MacEwen and Shands who first paid attention to the importance of correcting outward torsion of the femur, and devised an oblique trochanteric osteotomy by which both the femoral neck-shaft angle and femoral torsion. The authors experienced a case of bilateral congenital coxa vara on whieh correction of both the femoral neck-shaft angle and femoral torsion was achieved by osteotomy-osteoclasis on the right side which had exhibited a relatively mild degree of coxa vara and by the pin and plaster technique on the left side which had exhibited a severe degree of coxa vara. Both sides were treated at the same time. Advantages and disadvantages of other standard methods were discussed and compared with our experience.
