Two Cases of Bleb-Related Endophthalmitis after Trabeculectomy with Mitomycin C

Abstract

Filtering blebs after mitomycin C trabeculectomy have been frequently described as thin, avascular, and to be likely to develop a aqueous leakage. An eye with these characteristics is said to be more prone to subsequent development of endophthalmitis. Late bleb-related endophthalmitis developed in two glaucoma patients with the age of 42 and 63 years at 39 and 20 months after mitomycin C trabeculectomy, respectively. Filtering blebs in both patients had been thin-walled and cystic blebs until the occurrence of endophthalmitis. At the onset of endophthalmitis both patients complained of decreased visual acuity with ocular pain and showed a milky-white bleb surrounded by intense conjunctival or subconjunctival antibiotics (vancomycin, ceftazidime or cefamezine, gentamicin) and corticosteroid (prednisolone or dexamethasone) led to resolution within a few weeks. The visual acuity of both patients recoverd to the previous level before the occurrence of endophthalmitis.
