Troublesome fate of the penile augumentation with artificial oil analogues

Abstract

Artifical oil analogues have been not uncommomly injected into the subcutaneous tissues of the penis by patient-self or quack in the Far East. But unfortunately, the penile augumentation using this method to satisfy the sexual vanity and to increase sexual pleasure of the partner, has been frequently frustrating by resulting in serious complications in the most of cases. We reviewed the records of twenty seven patients who underwent the surgical correction for the severely complicated penile augumentation with artificial oil analogues from July 1981 to March 1991. 1. The age distribution on the time of injection was between 21 and 50 years old with the highest incidence at 3rd decades(40.7%). 2. The injected oil analogues were paraffin in 23 cases(85.2%), vaseline in 3(11.1%) and hydrocortisone ointment in one(3.7%) 3. The period of onset of underirable results, which included skin necrosis, ulceration, abscess, painful erection, difficult vaginal intromission and much more micturition difficulty, was between 2 weeks and about 10 years(ave. 26.4 months). 4. The extent of granuloma were confined within penile shaft in 18 cases(66.7%), spread to pubic area in 7(25.9%) and spread to pubic area scrotal sac in 2(7.4%). 5. The histopathological finding were commonly revealed extensive small ovoid or round lipid vacuoles and numerous foreign body granulomas with heavy infiltration of lymphoid cells and lipid laden histiocytes in the dermal stroma and partially upto the epidermis. 6. The surgical modalities for covering the bare penile shaft after complete excision of the granuloma were primary repair with scrotal skin only or with remnant penile skin in 21 cases(77.8%), staged scrotal embedding and flap technique in 5(18.5%) and scrotal remodelling in one(3.7%).
