Patient-Partner Satisfaction with Penile Prosthcsis in Impotence of Various Etiologies

Abstract

PURPOSE:The penile prosthesis has become an accepted therapeutic option for the treatment of impotence of various causes. The present study was undertaken to understand patient-partner satisfaction with penile prostheses and to evaluate any difference in satisfaction according to the etiology of impotence.
PATIENTS AND METHODS
A total of 47 patients with organic erectile dysfunction (average age 43 years) were treated with penile prostheses between January 1990 and December 1996. Every patient was evaluated with various diagnostic approaches, and the principal etiologies of the impotence were determined to be vasculogenic (51.1%), neurogenic (25.5%), and diabetogenic (17.0%), The types of prosthesis implanted were AMS 700 CXM (n=23), AMS 600 (n=17), mentor malleable (n=4), Mentor Alpha- (n=2), and AMS Hydroflex (n=1). The mean follow-up period was 29 months. The major motive for prosthesis implantation was poor vaginal intromission (51.1%), early detumescence (34.0%), dissatisfied sexual partner (10.6%), penile curvature (2.1%), or priapism (2.1%).
RESULTS
The rate of satisfaction with the degree of penile erection was 83.3% in both vasculogenic and neurogenic impotence and 87.5% min diabetogenic impotence. According to the cause of the impotence. According to the cause of the impotence (vasculogenic, neurogenic, and diabetogenic), the rate of patient satisfaction was 79.2%, 83.3%, and 75.0%, respectively, and the partner satisfaction rate was 75.0%, 75.0%, and 62.5%. Coital frequency increased in 29.2%, 16.7%, and 25.0% of the couples, respectively, and did not change in 66% of the total series. The rates of prolonged ejaculation were 16.7%, 8.3%, and 12.5%, but the ejaculation latency of most patients (80.9%) was not changed. Most patients (87.5%, 75.0%, and 75.0^, respectively, in the three groups) would recommend a penile prosthesis to someone else.
CONCLUSION
There were no major differences in satisfaction with a penile prosthesis according to the cause of the impotence. The implantation of a penile prosthesis is a safe and effective method for the treatment and rehabilitation of patients with erectile dysfunction of any origin.
