Intradural-Extramedullary Spinal Cavernous Angioma as a Cause of Subarachnoid Hemorrhage

Abstract

Two cases of intradural extramedullary cavernous angiomas are represented. The special features of these cases are noted: the acute clinical onset due to the manifestation as bleeding from this lesion rather than compression of spinal cord or spinal nerve root is emphasized. Both of them showed the symptom of subarachnoid hemorrhage with sudden headache and neck stiffness. In one case, a 33-year-old man complained of a sudden low back pain and the lesion is located at lumbar level, and in another case, a 61-year-old man complained of a recurrent headache with the lesion at cervical level. Magnetic resonance imaging (MRI) of spinal cord was diagnostic in both of them. Both of them exhibited characteristic gross and microscopic features indicating hemorrhage. The diagnostic, therapeutic aspects of cavernous angioma and a rare but possible cause of subarachnoid hemorrhage from cavernous angioma are discussed.
