Percutaneous Drainage and Alcohol-Retention Sclerotherapy for Two Adjacent Hepatic Cysts Causing Intra-Hepatic Bile Duct Dilatation

Abstract

Simple hepatic cysts are usually asymptomatic and found incidentally. In most cases, no treatment is required, although various management techniques with different clinical features and advantage have been developed. As one of the nonsurgical treatments for hepatic cyst, percutaneous drainage and sclerotherapy are often applied. This study presents a case of a 67-year-old male with cholestatic liver enzyme abnormality. Two adjacent hepatic cysts were shown to be causing compression of the liver parenchyma and intra-hepatic bile duct. Ultrasonography and computed topography revealed two large hepatic cysts and intrahepatic duct dilatation. After ultrasonography-guided percutaneous drainage of one of two larger cysts, alcohol-retention sclerotherapy was performed twice on two consecutive days. At five days after the initial therapy, the larger hepatic cyst was decreased from 10 × 8.3 cm to 4.4 × 1.7 cm and the cholestatic liver enzyme abnormality was normalized.
