A Clinico-Pathological Analysis of Superior Vena Caval Obstruction in 21 Cases and its Review

Abstract

Authors have made an analysis and review of the 21 patients of superior vena caval obstruction whose diagnosis have been confirmed either by clinical with or without pathological evidences at Yonsei Medical Center during past 14 years. The following results have been obtained : 1. Sex ratio of male and female was 9. 5: 1. The most patients were in the age group between the 5th and 7th decades(85. 7%) and the average age of them was 51.8 years of age. 2. The most common symptoms and signs were edema, venous dilatation of the face, neck, chest and upper extremities(100% ), dyspnea (71. 4% ), cough (42. 9%), flushing (28. 6% ) hemoptysis (19% ) and hoarseness(9.5% ). 3. The final diagnosis have made by clinical evidences, bronchoscopy and biopsy, biopsy of the supraclavicular lymph node, and open thoracotomy. Thirteen cases of patients were proved histopathologicallY (81. 3%) : 8 cases (61. 6% ) of bronchogenic carcinoma, 4 cases (30. 8% ) of lymphoma and 1 case of thrombophlebitis. 4. ln response of treatment, favorable results were observed in 5 cases (100% ) with combination of irradiation and chemotherapy, 5 cases(71. 4% ) with irradiation therapy alone, and 2 cases (66. 7%) with chemotherapy alone. One case caused by thrombophlebitis was completely cured after resection and ligation of vessels through the thrombophlebectomy.
