Three Cases of Traumatic Bone Extrusion of Upper Extremity

Abstract

In war time, traumatic bone extrusion is very common but it is relatively rare in civil life. Extruded bone loss in the limbs comes to occur, because the industry has rapidly developed and traffic means has gradually become highspeed recently. The auther has observed 3 cases of traumatic bone extrusion in civil life. All 3 cases occured in young boys. In case 1, 6 cm. of ulnar shaft was extruded, in case 2, 5 cm, of humeral shaft and In case 3, 6 cm. of distal radial shaft. In case 1, extruded bone was washed in soap and zephiran chloride solution, and in case 2, extruded bone was boild after washing. During operation, the fracture sites in cases 1 and 2 showed that the periosteum of the affected lesion was relatively intact, but in case 3 the periosteum was not visible at all. In cases 1 and 2, the extruded bone was replaced within 6 hours and 3 days of the injury respectively but in case 3, the missing part of radins was replaced by autogenous fibular bone 24 years after the childhood injury. Follow-up study showed good union 3 months after replacement in cases 1 and 2 and almost normal function was obsained. In cases 3, union was more slow than cases 1 and 2, but finally bony union took place 5 months after replacement. In case 2 roentgenogram of 5 months after replacement showed complete consolidation of the fracture, in case 1, roentgenogram of 2 years after replacement showed complete consolidation of the fracture site.
