Stress Urinary Incontinence: Comparison of Transperineal Ultrasonography and Cystography

Abstract

PURPOSE
To determine the effectiveness of transperineal ultrasonography (US) compared with cystography in Blaivas classification of stress urinary incontinence(SUI) MATERIALS & METHODS: We compared trasperineal US and cystography in 26 patients with SUI. US was performed sith a convex 7~4 MHz trasducer placed in the perineum in a sagittal orientation so as to obtain a whole view of the bladder, bladder base, urethrovesical junction, and the symphysis pubis. Descent of bladder neck was measured by subtracting the distance from the trasverse plane of symphysis pubis to the trasverse plane of bladder neck at rest from an similar distance on straining.
RESULTS
The concordance rate of Blaivas classfication of the SUI between the transperineal US using Valsalva maneuver and the cystography was 76.9%(20/26) and the rate between transperineal US using coughing strain, 4 patients by cystography, and 2 patients by transperineal US using Valsalva maneuver.
CONCLUSION
As a radiological study for diagnosis and Blavas classification of SUI, Transperineal US using coughing strain was more useful than cystography or transperineal US using Valsalva maneuver.
