A Case of Tuberculosis Cutis Orificialis

Abstract

Tuberculosis cutis orificialis is a rare form of tuberculosis of the mucous membranes and the skin of the orifices due to autoinoculation of mycobacteria from progressive tuberculosis of internal organs. We report herein a case of tuberculosis cutis orificialis in a 43-year-old man. He presented with a 2-year history of a large, painful, non-healing ulcer on the scrotum and perianal area. Histopathologic examination of the perianal ulcer revealed tuberculoid granulomas with Langhans' giant cells in the dermis and numerous acid-fast bacilli in Ziehl-Neelsen stain. Chest roentgenogram disclosed active pulmonary tuberculosis in both upper lung fields. Intradermal test with PPD showed negative result.
