Abstract

We report a case of priapism secondary to bilateral rupture of the cavernosal arteries following perineal blunt trauma in a 42 year-old man. He complained of painless and persistent partial erection sufficient to have sexual intercourse. Non-ischemic high-flow priapism attributable to artetial rupture was confirmed by arterial blood gas analysis and duplex ultrasonography. Bilateral arterial leakage, more severe on the right side than on the left, was revealed by pedendal selective angiography. Emergency embolization of the right pudendal artery resulted in immediate penile detumescence. We plan to evaluate the status of the left cavernosal artery, erectile potency and the expected penile fibrosis.
