Reversible Acute Renal Failure Associated with Rifampicin: Report of a Case

Abstract

A 52-year-old male started to take rifampicin, 450 mg twice weekly, with isoniazid, 400 mg per day, for the treatment of pulmonary tuberculosis. Half an hour after the 3rd dose, abrupt onset of fever, acute GI upset and cyanosis developed. During the following 11 days, the patient was oliguric. Administration of rifampicin, 450mg daily, with other antituberculsis drugs 5years previously, was not eventful. A renal biopsy revealed an interstitial nephritis. The patient received prednisolone, 30mg per day, and three hemodialyses and recovered from an acute renal failure. We feeI that rifampicin induceda hypersensitivity reaction. In view of its frequent use, the side effect of rifampicin has been reviewed.
