Slit-lamp photographs showed severe bilateral protrusion of the upper and lower lacrimal puncta (arrowheads). An increased tear meniscus height was also noted after fluorescein staining (A, arrow). Both upper lacrimal puncta protruded 2.0 mm in an inferolateral direction. Both lower lacrimal punctum protruded 1.5 mm in an upward direction. Intraoperative punctoplasty and findings at 2 weeks postoperative. (C) A circumferential incision was made from beneath the punctal orifice (arrowhead) to the protrusion base (arrow). A partial thickness wedge of the conjunctival epithelium and stroma was then excised. (D) The proximal and distal ends of the palpebral conjunctiva surrounding the incision were sutured radially using 8-0 polyglactin is correct. (E) At 2 weeks postoperatively, the tear lake was markedly reduced (asterisk). (F) The remaining suture material in the upper punctum of the right eye was observed on slit-lamp examination (arrows).
