A Case of Reflux Esophagitis with Hiatal Hernia Treated by the Belsey Mark IV Operation

Abstract

Reflux esophagitis (RE) is a common disease in Western countries. Recently, the incidence of RE is increasing in Korea, but the disease is rarely treated here through operations. Herein, we report a case of RE treated by the Belsey-Mark IV operation. The patients was a 62-year-old woman with a long history of anterior chest pain and heartburn. Endoscopy revealed severe RE with hiatal hernia and ambulatory 24-hour pH monitoring revealed severe gastroesophageal reflux. We failed to manage her symptoms using a double dose proton pump inhibitor and decided to perform surgical therapy. After the operation, the patient had difficulty in belching and felt epigastric discomfort. However, These symptoms improved with conservative management, and she is still comfortable without recurrence.
