Brainstem Auditory Evoked Potentials in Acoustic Neuromas and other Cerebellopontine Angle Tumors

Abstract

Brainstem auditory evoked potentials(BAEP) and clinical features were studied in 12 patients with acoustic neuromas and in10 patients with non-acoustic cerebellopontine angle(CPA) tumors. In acoustic tumor group, BAEP fundings were characterized by poor wave formation in 3 cases, well-defined wave I with indistinct or absent following waves in 6 cases, and prolonged interpeak latencies(lPL) in 3 cases on the affected sides. In non-acoustic tumor group poor wave formation in 1 case, well-defined wave I with absent following waves in 2 cases, and prolonged IPL in 7 cases were found on the affected sides. In each group, IPL prolongation was observed in 5 cases on the unaffected sides. BAEP findings tend to be more often abnormal in acoustic tumors than in non-acoustic CPA tumors Chief complaints were auditory symptoms(hearing loss or tinnitus) in 10 headache in 1 and dizziness in 1 in acoustic tumor group: auditory symptoms in 3, headache in 2, facial sensory change in 2, facial pain in 1, ataxia in 1 and vertigo in 1 in non-acoustic CPA tumor group Cranial nerve involvement was VIII in 12, V un 9, IX or X in 4, VII in 2. VI in 1 and XII in 1 in acoustic tumor group: VIII in 8, V in 9, VII in 5, IX or X in 3, II in 1. VI in 1 and XII in 1 in non- acoustic CPA tumor group. Increased DTR in 2 and cerebellar dysfunction in 9 were found in acoustic tumor group and in 3, 7 respectively in non-acoustic CPA tumor group. There was electrophysiological improvement in postoperative BAEPs in 3 cases. The postoperative BAEP study mightbhe the good tool of follow-up.
