Aneurysms of Proximal(A1) Segment of Anterior Cerebral Artery

Abstract

This study reviewed aneurysms of the proximal segment(A1) of the anterior cereral artery in 16 patients(5 men and 11 women) and their surgical, angiographic, and clinical management. The incidence of A1 segment aneurysm among a total of 837 aneurysm cases treated was 1.9%(16:837). All aneurysms were saccular, and multiple aneurysms occurred in 4 patients(25%) of the 16 cases:in 2(50%) there was bleeding from A1 aneurysm. In 16 of these cases, the aneurysms arose from the proximal 1/3 segment(5 cases), middle 1/3(8 cases), or distal 1/3(3 cases). When performing radical surgery, it is very important to recognize the characteristics of perforators in A1 segment with multiple and hidden cortical branches for improving prognosis after operation.
