Ultrasonographic Diagnosis of Macrofollicular Variant of Papillary Thyroid Carcinoma

Abstract

Macrofollicular variant papillary thyroid carcinoma (MFV-PTC) is rare variant of PTC, and is a well-differentiated thyroid carcinoma with large, follicular architecture and with nuclear characteristics of PTC. Fine needle aspiration (FNA) cytology is not helpful in the diagnosis in most of the cases, as the cytological features and ultrasonography may be difficult to distinguish from those of adenomatous goiter and macro-follicular adenoma. Here, we represent a case of MFV-PTC which has benign ultrasonographic findings and indeterminate cytology and review of the literatures. A 58-year-old woman referred for evaluation of enlarging palpable mass of right thyroid gland. A thyroid ultrasound revealed well defined isoechoic nodular lesion. FNA cytology showed suspicious for follicular neoplasm. Total thyroidectomy was done and the final diagnosis was MFV-PTC with capsular invasion.
