Abstract

Pulmonary lobar agenesis is a rare entity of developmental anomalies which is usually unilateral. To the best of our knowledge, bilateral lobar agenesis has not yet been reported in the literature. We report a case of lobar agenesis of both upper lobes in a 40-year-old female.

Figure

  • Fig. 1 A. Chest radiograph shows decreased lung volume of left lung with indistinct cardiac border, mediastinal shifting to left side and flattening of left hemidiaphragm. B. Serial axial chest CT images show highly positioned interlobar fissure (arrowheads) and superior segmental bronchus (open arrowheads) of right lower lobe which passes through the upper portion of right lung. The right middle lobe bronchus (black arrow) also passes upper portion of right lung. So the right upper hemithorax is consist of some portion of right middle lobe and superior segment of right lower lobe. Also note decreased left lung volume with nonvisualized left major fissure. C. Reformatted slap minIP coronal image shows absence of bilateral upper lobe bronchi. The superior segmental bronchus (open arrowhead) of right lower lobe passes through the right upper lung, considering serial axial images. D. Reformatted oblique axial image of mediastinum shows hypoplastic left pulmonary artery (black arrowhead) and dilated right pulmonary artery (white arrowhead).
